Cortisol is often called the “stress hormone,” but its role in the body is far more complex. During fasting, cortisol does change — but not in the way many people fear. Understanding how cortisol works helps explain why blood sugar can rise during fasting and why short-term stress is different from chronic stress.

Let’s break down what actually happens.

What Is Cortisol?

Cortisol is a hormone produced by the adrenal glands in response to signals from the brain. This system is known as the HPA axis (hypothalamus–pituitary–adrenal axis).

Cortisol’s primary roles include:

  • Maintaining blood glucose levels
  • Supporting energy availability
  • Regulating inflammation
  • Helping the body respond to stress

Cortisol follows a natural daily rhythm. It is highest in the early morning (which helps you wake up) and gradually declines throughout the day.

It is not inherently harmful. It is essential for survival.

Does Fasting Increase Cortisol?

Short answer: Yes — slightly and temporarily.

During fasting, insulin levels fall. As the body shifts from storage mode to energy release mode, several hormones rise to maintain stable blood sugar. Cortisol is one of them.

Cortisol helps by:
